Qualifications

• Maintain updated state related Clearances.
• Complete training courses as required or amended by program regulations.
• Valid Driver's License including personal vehicle insurance coverage for liability and current vehicle registration.

Responsibilities

• Identify the disabilities and accompanying needs of participants and help them achieve their goals.
• Develop and modify each participant's plan annually or after significant changes.
• Coordinate with healthcare and support services to ensure seamless delivery of care.
• Build and maintain professional relationships with participants and families.
• Communicate with providers and stakeholders to coordinate services across disciplines.
