Job summary

Vocational Training Center (VTC) of Fargo, ND is seeking a Case Manager (QIDP) to lead the person-centered service plan process within the Program Services team. You will develop and monitor support plans, assess progress, and coordinate meetings with the support network to advance individual objectives.

Ideal candidates hold a Bachelor's degree in social work or related field and have experience with individuals with intellectual and developmental disabilities, mental health, or related areas.

Qualifications

• Bachelor's degree in social work, human services or related field.
• Minimum one year experience working with individuals with intellectual and developmental disabilities, mental health or related field.
• Must complete the competency-based modules as dictated by the North Dakota Department of Human Services, Developmental Disabilities Division.
• Strong interpersonal, oral, and written communication skills.

Responsibilities

• Developing, monitoring and reporting on support plans
• Conducting assessments and creating progress reports
• Coordinating and facilitating support network meetings
• Traveling to local community sites to assess progress of supported individuals' personal objectives
• Training Direct Support Professionals on methods, strategies and techniques relative to supported individuals’ programs and active support
