Job summary

Qualifications

• Bachelor's degree required in social work, psychology, rehabilitation counseling or related field.
• At least 1 year of experience in the human services field preferred.
• Clean driving record with no more than two points; ability to pass a criminal background check.

Responsibilities

• Develops an Individual Rehabilitation Plan with the person served to identify case management needs.
• Documents communications and progress notes in a timely, accurate manner.
• Schedules and conducts home visits at least twice per month to assess needs.
• Provides linkages to resources, entitlements, and supports for community integration.
