Our robust training program allows staff members to develop and grow in the position and provides the basis for upward mobility within the agency.**Responsibilities of the Case Manager*** Develops an Individual Rehabilitation Plan (Treatment Plan) with the person served to identify case management needs in the areas of advocacy, coordination of services and resource procurement.* Documents communication and interactions with the person served in progress notes in a timely manner. Assures that the assigned cases and all documentation are in compliance and accurate according to agency, COMAR, and CARF standards.* Schedules and conducts home visits with persons served to monitor their condition and to assess their needs at least twice per month.* Provides linkages and access to the necessary resources, entitlements and supports to ensure successful community integration.## Qualifications**Qualifications of the Case Manager*** A Bachelor's Degree in Social Work, Psychology, Rehabilitation Counseling, or a related discipline is required.* At least one year of experience working in the human services field preferred* Candidate must have a clean driving record with no more than two points and the ability to pass a criminal backgroundOur company offers a dynamic and collegial work environment, ongoing opportunities for professional growth, and generous benefits, including 3 weeks paid vacation, up to 500 hours of sick leave, 9 paid holidays and 2 floating holidays (bonus days).
