Independence Center in Illinois seeks a full-time Case Manager (36 hours/week) with a Bachelor's degree in a human services field to perform mental health assessments, develop treatment plans, and link clients to community resources.

Responsibilities include case management, referrals for entitlements, coordinating medical and social services, conducting outreach, leading life skills groups, and maintaining documentation; candidate must be team-oriented, reliable, and proficient with Microsoft

Qualifications

• Bachelor's degree in counseling, rehabilitation counseling, social work, education, vocational counseling, psychology, pastoral counseling, family therapy, or related human service field.
• Two years of supervised clinical experience in a mental health setting or CRSS certification.
• Experience in crisis intervention & de-escalation.
• Valid driver’s license.

Responsabilités

• Conduct and document mental health assessments and treatment plans.
• Conduct case management linking members to community resources and referrals to meet needs.
• Provide community support services and assist residents in achieving rehabilitation, resiliency, and recovery goals.
• Coordinate, transport, and participate in appointments with mental and physical health providers.
• Conduct non‑medical assessments to provide referrals and resolve concerns.
• Provide outreach via phone, email, in person, and text to reduce barriers to treatment.
• Facilitate educational and therapeutic groups in life skills, coping, and conflict management.
• Carry a caseload of clients providing supportive counseling and teaching coping skills.
• Monitor client's medications and attend appointments.
• Help clients find housing and establish routines.
• Provide crisis planning and supporting interventions.
• Other duties as assigned by supervisor.
