The Gathering Inn in Roseville, CA is seeking a dedicated Case Manager to support unhoused individuals toward stability and housing. You will conduct needs assessments, develop individualized care plans, and coordinate access to services using HMIS.

Responsibilities include documenting interactions, coordinating with partners, leading classes, maintaining confidentiality, and participating in trainings such as CPR and de-escalation. The role is full-time, non-exempt with a $28/hour compensation.

Qualificações

• Bachelor's degree with one year experience or an equivalent blend of educational qualifications and practical experience.
• Computer skills for documentation and record-keeping.
• Ability to pass a Live Scan background check.

• Maintain accurate and up-to-date records of guest interactions, services provided, and outcomes, utilizing HMIS
• Coordinate access to service providers and resources, ensuring eligibility, follow-through, and seamless delivery of support
• Work collaboratively with community partners to advocate for guests, schedule appointments, arrange transportation, and make referrals
• Facilitate and lead assigned classes or programs for guests
• Uphold guest confidentiality and data privacy standards
• Address behavioral concerns calmly and respectfully, maintaining safety and harmony
• Participate in staff meetings, in-service training, and other professional development opportunities, including De-Escalation, Motivational Interviewing, CPR, and First Aid

Originally a winter-only shelter serving 40 individuals nightly, we have evolved into a multifaceted organization with multiple locations, serving nearly 500 individuals and families daily. What You'll Do

As a Case Manager, you play a vital role in helping guests move toward stability and self-sufficiency. You will:

• Coordinate access to service providers and resources, ensuring eligibility, follow-through, and seamless delivery of support
• Work collaboratively with community partners to advocate for guests, schedule appointments, arrange transportation, and make referrals
• Facilitate and lead assigned classes or programs for guests
• Uphold guest confidentiality and data privacy standards

Schedule: Tuesday - Saturday, 8:00 AM- 4:30 PM

Status: Full-Time, Non-Exempt

Compensation: $28/hour

Requirements

Minimum Qualifications

• A bachelor's degree with one year experience or an equivalent blend of educational qualifications and practical experience is necessary. This could include a high school diploma and a minimum of five years of relevant experience. Alternatively, candidates with an associate's degree and three years of experience will also be considered.
• Computer skills for documentation and record-keeping.
• Ability to pass a Live Scan background check
