Job summary

The Doe Fund Inc in Brooklyn, NY is seeking a high-energy, self-motivated Case Manager to support clients through the Ready, Willing & Able program, based at the 510 Gates Avenue Program in Brooklyn.

You will manage caseloads, coordinate referrals, document progress, and collaborate with DHS and staff to meet program goals. The role requires a HS diploma and 2+ years in human services.

Qualifications

• A High School Diploma is required. Bachelor’s degree in Psychology, Sociology, Counseling, Social Work, or other Human Services concentration preferred.
• At least 2+ years of experience in Human Services/Case Management.
• Experience working with challenging high-service needs populations preferred.
• Strong interpersonal and written/verbal communication skills required.
• Working knowledge of MS Word, MS Excel, MS PowerPoint, and Google.

Responsibilities

• Manage a caseload of clients; monitor progress toward goals in service plans.
• Research and coordinate access to Mental Health, Benefits, and Primary Care services.
• Communicate with DHS and other agencies as needed.
• Collaborate with clients and staff to achieve program goals (employment, housing, graduation).
• Organize and facilitate caseload and other groups on various topics.
• Maintain current, accurate documentation of trainee progress in charts and databases.
• Provide crisis intervention to clients to resolve issues.
• Perform other duties as assigned.
