Job summary

The Doe Fund seeks a devoted Case Manager/Mental Health Counselor for its Crystal Towers Residence in Brooklyn, NY. The role focuses on delivering supportive case management for HASA-funded residents and ensuring access to stable housing and health services.

The ideal candidate holds a Master’s Degree in Social Work or related field, with strong clinical, communication, and organizational skills and at least 5 years in the mental health sector.

Qualifications

• Master's Degree in Social Work or related field required or preferred.
• At least 1 year experience with homeless individuals or HIV/AIDS populations.
• At least 5 years of experience in the mental health field.
• Knowledge of NYC Metro area benefits, HIV/AIDS, psychiatric diagnoses and substance abuse.

Responsibilities

• Maintain a caseload and provide regular case management.
• Maintain resident records in compliance with standards.
• Develop and implement Individual Service Plans.
• Provide counseling and crisis intervention as needed.
• Coordinate with healthcare providers and community services.
