Job summary

The Doe Fund is seeking a Case Manager/Mental Health Counselor for the Crystal Tower House Permanent Supportive Housing Program in Brooklyn, NY. This full-time role provides long‑term residential care and 24‑hour support for homeless men and women living with HIV/AIDS, focusing on independence and security.

You will maintain caseloads, develop service plans, link residents to healthcare and benefits, provide counseling, and collaborate with community services.

Qualifications

• Master's degree in social work or related field.
• Licensure in clinical social work or mental health counseling is preferred.
• Minimum experience in mental health field and working with vulnerable populations.

Responsibilities

• Maintain a caseload and provide regular case management services to HASA-funded residents.
• Develop and implement individual Service Plans and assist residents with goals toward self-sufficiency.
• Assist residents in securing and maintaining benefits and linking to healthcare providers.
• Provide counseling and crisis intervention as necessary.
