Job summary

The Doe Fund in Bronx, NY is seeking a Case Manager for the new permanent supportive housing program serving homeless individuals and families living with HIV/AIDS. You will provide supportive case management, connect residents with services, and promote independence.

Responsibilities include maintaining records, developing service plans, coordinating healthcare linkages, and assisting with benefits, while upholding confidentiality and ethical standards.

Qualifications

• Bachelor’s Degree in Social Work or relevant field.
• Minimum of three years of post-Master's experience.
• Ideal candidate must have working knowledge of HIV/AIDS, substance abuse, mental illness, and homelessness.
• Excellent case management skills, including knowledge of benefits in the NYC Metro area.
• Sensitivity to community issues and needs; strong communication and integrity.

Responsibilities

• Maintain a caseload and provide supportive case management services to these individuals
• Maintain resident records in compliance with agency and funding standards
• Conduct and review assessments
• Develop and implement individual Service Plans and assist residents with achieving their goals geared towards self-sufficiency
• Monitor and ensure residents are linked to healthcare providers
• Provide counseling and crisis intervention as necessary
• Assist residents in securing and maintaining benefits
• Maintain a liaison with community services including hospitals, substance abuse treatment programs, HRA, and mental health programs
• Facilitate groups and activities that promote health and independent living skills development
• Monitor and assist residents with rent payments and budgeting
• Educate residents about approaches to mental health
• Attend ongoing trainings to enhance the services provided to the residents
• Assist with completion of reports as needed
• Other duties as assigned

The Doe Fund's newest permanent supportive housing program for homeless individuals and families living with HIV/AIDS. The program is designed to provide long‑term residential care and twenty‑four‑hour support services in a community environment that maximizes residents' independence while ensuring their physical and emotional security and well‑being. This 151‑unit residence will provide housing for 31 individuals referred by the New York City HIV/AIDS Services Administration (HASA), and the remaining 120 units will be apartments for moderate‑income households.

The Case Manager is responsible for providing supportive case management services to the residents referred by HASA. The successful candidate will have strong clinical and communication skills along with experience working with the HIV/AIDS population.
