Job summary

The Doe Fund Inc. is seeking a Case Manager for a new 90-unit permanent supportive housing project serving homeless individuals, including veterans with severe mental illness or past substance use.

You will provide supportive case management and counseling to residents referred by HRA, building independence in a secure, community-based setting. Qualifications

• Bachelor's degree in Social Work or relevant field required.
• At least 3 years of experience with homeless individuals with serious mental illness and/or substance use disorders.
• Experience with trauma settings and evidence-based practices.
• Strong clinical and case management skills in NYC metro area.
• Excellent interpersonal and written communication skills.

Responsibilities

• Maintain a caseload of individuals with serious mental illness or substance use disorders.
• Develop and facilitate therapeutic groups for residents.
• Maintain resident records in compliance with standards.
• Conduct and review assessments and develop Service Plans.
• Assist residents with achieving goals toward self-sufficiency.
• Monitor links to treatment and services; provide counseling as needed.
• Assist with rent payments, budgeting, and benefits access.
• Coordinate with hospitals, treatment programs, HRA and mental health services.
