Job summary

Eliot Community Human Services in Lowell is seeking a Community Supports Case Manager to empower adults with developmental disabilities. This role focuses on assessments, service planning, and coordinated care to secure housing, employment, and community participation.

Responsibilities include ISP/POC development, goal setting with individuals, and strong collaboration with families and providers. The position is based in a community setting with standard hours and a near-term salary of $20

Qualifications

• Bachelor’s degree or prior experience in case management.
• Experience working with intellectually disabled or brain injured adults.
• Ability to manage a client caseload in an outreach setting.
• Must possess a valid driver’s license and maintain a personally owned vehicle.
• Ability to work a community based position serving Lawrence, Lowell, Burlington and Arlington areas.

Responsibilities

• Assist in completing assessments and ISP/POC for assigned cases per DDS/MRC policies.
• Ensure individuals help identify and develop their own goals.
• Document daily progress notes for all individuals per Eliot policy and DDS/MRC requirements.
• Develop and maintain positive professional relationships with individuals, families, and providers; maintain schedules and timeliness.
• Coordinate service delivery for cases assigned (case conferences, housing, SSI) and advocate with outside agencies.
• Provide service linkages and schedule appointments with individuals as needed.
