Qualifications

• Experience working with individuals with ID/DD.
• Spanish speaking is a plus.
• Must have Good communication and documentation skills.

Responsibilities

• Promote health and safety of individuals.
• Provide supervision, training and coaching across settings.
• Develop, write and present ISP plans.
• Perform case management and maintain confidential files.
• Provide transportation as needed and use mileage reimbursement.
