Job summary

Eliot Community Human Services in Massachusetts seeks a Community Supports Case Manager to support adults with developmental disabilities. You will help develop ISP/POC, advocate for services, and assist with housing and community integration.

The role requires coordinating care, documenting progress, and collaborating with DDS/MRC policies while building strong relationships with individuals, families, and providers. Schedule: Tue–Sat, 9am–5pm or 10am–6pm; Hourly rate: $20.

Qualifications

• Bachelor's degree or related experience in case management.
• Experience with intellectually disabled or brain injured adults.
• Must possess a valid driver’s license.
• Roles serve Lawrence, Lowell, Burlington and Arlington areas.

Responsibilities

• Assist in completing assessments and ISP/POC for assigned cases.
• Ensure individuals help identify and develop their own goals.
• Document daily progress notes per Eliot policies.
• Maintain professional relationships with individuals, families and providers.
• Coordinate service delivery, case conferences, and advocate with outside agencies.
• Provide service linkages and schedule appointments as needed.
