Job summary

Eliot Community Human Services is seeking a Community Supports Case Manager to support adults with developmental disabilities in the Lowell area. The role involves assessments, ISP/POC development, goal setting, and coordinating services with DDS and community providers.

Schedule is Tue-Sat, 9am-5pm or 10am-6pm; hourly rate is $20 USD. Benefits include paid time off, comprehensive benefits, retirement with match, wellness resources, and professional growth opportunities.

Qualifications

  • Bachelor’s degree or equivalent experience in case management.
  • Experience working with intellectually disabled or brain injured adults.
  • Ability to manage a client caseload in an outreach setting.
  • Must possess a valid driver’s license and maintain a personally owned vehicle.
  • Ability to work in a community-based position serving Lawrence, Lowell, Burlington and Arlington areas.

Responsibilities

  • Assist in completing assessments and ISP/POC for assigned cases.
  • Ensure individuals are involved in identifying and developing their own goals.
  • Document daily progress notes in accordance with Eliot Policy and DDS/MRC requirements.
  • Develop and maintain positive working relationships with individuals, families and providers.
  • Coordinate service delivery for cases assigned, including housing and SSI advocacy.
  • Provide service linkages to other providers as necessary.
  • Schedule appointments for/with individuals.

Eliot’s Developmental Disabilities division is seeking a Community Supports Case Manager. This division offers support and opportunities for individuals to enrich their lives, obtain housing and employment, develop personal skills and competencies, and enjoy a full range of community activities while developing a network of caring relationships. The ideal candidate will have experience working with adults with developmental disabilities and will have the ability to advocate for community supports and services on behalf of the individuals.

Responsibilities
  • Assist in completing assessments and Individual Service Plans(ISP)/Plans of Care(POC) for assigned cases in a timely manner and as indicated by DDS and MRC policies and standards.
  • Ensure individuals are involved in identifying and developing their own goals.
  • Document daily progress notes for all individuals in accordance with Eliot Policy and DDS/MRC documentation and billing requirements.
  • Develop and maintain a positive and professional working relationship with individuals, families and providers. This includes maintaining a set schedule with individuals, timely communication and follow up on issues and requests and being on time for appointments, meetings and activities.
  • Coordinate service delivery for cases assigned i.e., case conferences, Section 8 Housing, SSI and advocate for the consumer with outside agencies and providers.
  • Provide service linkages to other providers as necessary.
  • Identify and schedule appointments, for/with individuals, as needed or required.
Qualifications
  • Bachelor’s degree or prior experience in/or exposure to case management
  • Experience working with intellectually disabled or brain injured adults
  • Ability to manage a client caseload in an outreach setting
  • Must possess a valid driver’s license and maintain a personally owned vehicle
  • Ability to work a community based position that services the Lawrence, Lowell, Burlington and Arlington areas
Schedule:

Tuesday-Saturday, 9am-5pm or 10am-6pm

Hourly Rate
$20—$20 USD
