Job summary

Health Connect America is seeking a Targeted Case Manager to provide best-practice case management services for children, families, and adults. You will ensure documentation meets payer, state, and COA standards while assisting in marketing new referrals.

Responsibilities include coordinating service plans with the treatment team, connecting families to community resources, and maintaining client engagement for up to 12 months with regular team participation and on-call support.

Qualifications

• Mississippi: Master’s degree in a human services, behavioral health, social work, counseling, or related field required.
• Mississippi: Must meet payer-required credentialing standards, which may include: Licensed Social Worker (LSW), or DMH-approved Mental Health Therapist credential, or Registered Nurse (RN).
• Mississippi: Must have 2 years of experience in mental health care.
• Florida: Must hold a Bachelor’s degree in a human services discipline, including but not limited to: Social Work, Psychology, Sociology, Counseling, etc.
• Florida: Must have experience working with children and families in case management or community resource capacity.

Responsibilities

• Communicate with customers and colleagues to answer questions, disseminate information, and address concerns.
• Develop service plans with input of the treatment team and the Clinical Supervisor.
• Connect families with needed community resources and follow up with clients and agencies to document use/success of referral.
• Complete documentation within established time frames.
• Meet with clients weekly or bi-weekly for up to 12 months.
• Attend bi-weekly Treatment Team meetings.
• Serve as a liaison with other professionals and agencies related to the client.
• Actively recruit new referrals to keep caseload full.
• Provide 24/7 on-call support for clients per program requirements.
• Ensure compliance with all state regulatory bodies.

Education

Bachelor’s degree in human services discipline

Master’s degree in human services, behavioral health, social work, counseling, or related field
