Job summary

Center for Family and Child Enrichment (CFCE) seeks a Targeted Case Manager to provide care coordination for children and families in behavioral health and child welfare programs. You will assess needs, develop service plans, and connect families with healthcare, education, and community resources.

You will collaborate with schools, providers, and agencies to promote client stability and safety while documenting progress and maintaining HIPAA compliance. A valid driver’s license is required.

Qualifications

• Bachelor’s Degree in Social Work, Psychology, Human Services, or a related field.
• Experience in case management, behavioral health, child welfare, or community-based services preferred.
• Knowledge of Medicaid-funded Targeted Case Management services preferred.
• Valid Driver’s License, current auto insurance, and access to a reliable vehicle.

Responsibilities

• Provide Targeted Case Management services to children and families per program and Medicaid requirements.
• Assess client needs and develop, implement, and monitor service plans.
• Coordinate referrals to healthcare, behavioral health, educational, and community resources.
• Advocate for client needs and support access to services.
• Maintain regular contact with clients, families, providers, schools, and community agencies.
• Monitor progress and update service plans; maintain case documentation.
• Collaborate with internal teams, schools, child welfare agencies, and external providers.
• Participate in team meetings, quality assurance activities, and program requirements.
• Protect client confidentiality and comply with HIPAA, Medicaid, TCM policies.

The Targeted Case Manager (TCM) at the Center for Family and Child Enrichment (CFCE) provides care coordination and supportive services to children and families involved in behavioral health, child welfare, and community-based programs. The Targeted Case Manager works with clients and families to identify needs, develop service plans, coordinate resources, and connect individuals with appropriate healthcare, behavioral health, educational, and community services.

Responsibilities include maintaining an assigned caseload, conducting assessments, developing and monitoring service plans, coordinating referrals, documenting services, and advocating for client needs. The Targeted Case Manager collaborates with families, schools, healthcare providers, child welfare agencies, and community partners to promote client stability, safety, and progress while ensuring compliance with Medicaid, TCM, and organizational requirements.

Some of the Functions Include:

• Provide Targeted Case Management services to children and families in accordance with program and Medicaid requirements.
• Assess client needs and develop, implement, and monitor individualized service plans.
• Coordinate referrals and connect clients and families with healthcare, behavioral health, educational, and community resources.
• Advocate for client needs and support access to appropriate services.
• Maintain regular contact with clients, families, providers, schools, and community agencies.
• Monitor client progress and update service plans as needed.Maintain accurate and timely case documentation, progress notes, and required records.
• Collaborate with internal teams, child welfare agencies, schools, and external providers to coordinate services.
• Participate in case staffing, team meetings, quality assurance activities, and other program requirements.
• Protect client confidentiality and comply with HIPAA, Medicaid, TCM, and organizational policies.
• Perform other duties as assigned.
• Bachelor’s Degree in Social Work, Psychology, Human Services, or a related field.
