Job summary

The Achievement Center of LECOM Health is seeking a Blended Case Manager to support youth with physical and behavioral health needs, coordinating services to ensure timely, appropriate care.

This role involves advocating for school placements, collaborating across programs, and working flexible hours, including on-call shifts. A relevant bachelor’s degree or equivalent plus driving access is required.

Qualifications

• Bachelor's degree or equivalent combination as described in the posting.
• Must have driver’s license and access to a reliable vehicle.
• Clearances and mandated reporter training within 30 days of hire.

Responsibilities

• Support physical and behavioral health needs of children/adolescents by identifying strengths and accessing resources.
• Advocate for school placements and knowledge of Special Education/IEP processes.
• Collaborate with other services to ensure needs are met.
• Work non-traditional hours and provide on-call rotation.
