Job summary

Health Connect America is seeking a dedicated Targeted Case Manager to deliver best-practice case management under payer, state, and COA standards. You will coordinate services, document timelines, and help families access community resources.

You will work with treatment teams to develop service plans, meet clients weekly or bi-weekly for up to 12 months, and participate in team meetings. We offer flexibility, comprehensive benefits, and growth opportunities in a values-driven organization.

Qualifications

• Bachelor’s degree in a human services discipline (Social Work, Psychology, Sociology, Counseling) required in Florida.
• Master’s degree in a human services, behavioral health, social work, counseling, or related field required in Mississippi.
• Must have 2 years of experience in mental health care (as applicable).
• Must hold credentialing standards (LSW, DMH-approved Mental Health Therapist, or RN) as payer requires.

Responsibilities

• Communicate with customers, colleagues, and other individuals to answer questions and address concerns.
• Develop service plans with input from the treatment team and Clinical Supervisor.
• Connect families with community resources and document referral outcomes.
• Complete documentation within established time frames.
• Meet with clients weekly or bi-weekly for up to 12 months.
• Attend bi-weekly Treatment Team meetings and coordinate with other professionals.

Education

Master’s degree in a human services, behavioral health, social work, counseling, or related field
