Job summary

Health Connect America seeks a Targeted Case Manager to deliver high-quality case management for children, families, and adults. The role ensures documentation meets payer, state, and COA standards, and supports marketing of referrals.

You will develop service plans with the treatment team, connect families to resources, and maintain timely documentation while meeting weekly or bi-weekly with clients for up to a year.

Qualifications

• State Specific Qualifications: Mississippi requires a Master's degree in human services, behavioral health, social work, counseling, or related field.
• Mississippi may require licensure or credentialing such as LSW, DMH-approved Mental Health Therapist, or RN.
• Must have 2 years of experience in mental health care.
• Florida requires a Bachelor's degree in a human services discipline and experience with children and families in case management or community resource capacity.

Responsibilities

• Communicate with customers, colleagues, and others to answer questions and address complaints/concerns.
• Develop service plans with input from the treatment team and Clinical Supervisor.
• Connect families with community resources and document use/success of referrals.
• Complete documentation within established time frames.
• Meet with clients weekly or bi-weekly for up to 12 months.
• Attend bi-weekly Treatment Team meetings.
• Serve as liaison with professionals and agencies related to the client.
• Actively recruit new referrals to keep caseload full.
• Provide 24/7 on-call support for clients per program requirements.
• Ensure compliance with state regulatory bodies.
