Catholic Charities Community Services is seeking a Case Manager/Family Development Coach to join our Washington Heights site. The role focuses on direct client support, crisis intervention, and coordinating services to help families access shelter, food, and benefits.

Strong trauma-informed practice and bilingual skills are required. The position is full-time, non-exempt, with a typical 9:00 AM–5:00 PM schedule, and involves field visits and community outreach to strengthen client networks and

Competenze

Bachelor’s degree preferred or Associate’s degree in human services with at least two years’ experience.
Bilingual Spanish/English required.

Provides engagement, assessment, service planning and follow-up using a strengths based and trauma-informed approach.
Provides crisis intervention in emergencies and connects clients to shelter, food, clothing, and benefits.
Responds to disasters and provides case management support.
Develops understanding of the social service system and navigates networks with clients.
Designs and provides peer support groups with community members.
Engages with families including parents, children and grandparents and builds support systems.
Conducts community outreach to build program capacity.
Maintains collaborative relationships with community agencies and Archdiocesan entities.
Enters information into Data Management Systems and Meets program deliverables with DYCD.
Gathers documentation for financial assistance and attends meetings.
Assists at other sites; participates in supervision and trainings.
