Job summary

Catholic Charities Community Services seeks a Case Manager/Family Development Coach to support immigrant families at the Alianza Dominicana Cultural Center in Washington Heights. You will assess needs, plan services, and connect clients to housing, food, and benefits using a strengths-based, trauma-informed approach.

Qualifications

• Bachelor’s degree preferred or Associate’s degree required in human services with at least two years’ experience.
• Bilingual Spanish/English required.
• Strong interpersonal, organizational, and communication skills.

Responsibilities

• Engage, assess, plan, and follow up with clients using a strengths based and trauma-informed approach.
• Provide crisis intervention for shelter, food, and basic needs; assist with financial support, food pantry, and benefits access.
• Respond to disasters with case management support as needed and navigate social service networks.
• Develop understanding of social service systems and partner with clients to access resources.
• Design and provide peer support groups in collaboration with community members.
• Provide services to the whole family and conduct community outreach to build program capacity.
• Enter information into Data Management System(s) and meet deliverables with DYCD; attend supervision and meetings.
