Qualifications

• Must be qualified to independently complete an assessment within the scope of practice.
• Active license for the respective discipline is required where applicable.
• CCM or ACM certification must be obtained within two years of starting the role.

Responsibilities

• Coordinate discharge planning with interdisciplinary team and plan contingencies.
• Complete case management documentation and addendums in patient records.
• Educate patients and families about rehabilitation and the Case Manager role.
• Attend ACT meetings to identify trends and reduce unnecessary transfers.
• Monitor regulations compliance, payor requirements, and documentation accuracy.
