Job summary

Encompass Health is seeking a Case Manager to coordinate patient care from admission to post-discharge within a national inpatient rehabilitation framework. You will lead interdisciplinary plan-of-care decisions and oversee discharge planning to ensure smooth transitions and exemplary patient outcomes.

Qualified candidates will demonstrate clinical assessment skills, proper state licensure, CCM/ACM certification or an approved timeline to obtain, and a strong preference for rehabilitation

Qualifications

• Must be qualified to independently complete an assessment within the scope of practice of their discipline.
• Licensure in state, if required, must be held.
• CCM® or ACM™ certification required OR to be obtained within two years.
• For nursing, RN licensure with BSN preferred; diploma acceptable where permitted by state.

Responsibilities

• Work with interdisciplinary team, guiding treatment plans based on patient needs and preferences.
• Coordinate with interdisciplinary team to establish tentative discharge plan and contingency plans.
• Participate in planning for and execution of patient discharge experience.
• Monitor patient experience: quality, timeliness, service appropriateness, payors, expectations.
• Facilitate team conferences weekly and coordinate all treatment plan modifications.
• Complete case management addendums and all required documentation.
