WES HEALTH SYSTEM in the United States seeks a BETCM Case Manager to support clients in accessing medical, social, educational, and other needed services. You will assess needs, develop service plans, and advocate for improved services and reduced stigma.

Role includes coordinating community resources, providing counseling, and maintaining case records. On-call availability and in-office outreach are required to support clients and crisis situations.

Qualifications

• Bachelor's or Master's degree in Human services or related field or equivalent.
• Registered Nurse (RN) qualification or clinical background.
• Equivalent combination of training and experience as approved.

Responsibilities

• Assist clients in accessing medical, social, educational, and supportive services.
• Develop service plans after assessing client history and life situation.
• Advocate to improve services and reduce stigma.
• Provide supportive counseling to clients and families.
• Coordinate with community resources and service providers.
• Maintain documentation of consumer contacts and service plans.
