Job summary

WCSI is seeking a full-time Targeted Case Manager to assess client needs, develop a detailed service plan, and coordinate a broad range of social services. The role serves clients from 3 years to senior ages, focusing on housing, education, and other social determinants of health.

Expect evenings, weekends, and on-call rotations as needed to support outreach and after-hours coverage. The position emphasizes documentation, collaboration with interdisciplinary teams, and engagement with community

Qualifications

• Bachelor’s degree in a social services related field is required.
• For option B, an Associate degree plus 2 years direct behavioral health experience and 2 years as a TCM Level 1 at WCSI.
• A high school diploma with related credits and two years of direct behavioral health experience plus two years as a TCM Level 1 at WCSI is acceptable.

Responsibilities

• Assess client needs using the Environmental Matrix and SBA.
• Develop, prepare and implement a comprehensive Targeted Case Management Service Plan.
• Monitor all services received by clients and coordinate resources.
• Provide crisis intervention and advocacy to clients.
• Maintain Targeted Case Management records and prepare for audits.
