Qualifications

• Bachelor's degree in a relevant field is required.
• Certification in TCM meeting Florida requirements.
• Minimum of one year of experience in case management or a related field.
• Bilingual candidates are a plus.

Responsibilities

• Assess and plan individualized service plans based on client needs.
• Coordinate access to mental health, housing, employment, and community resources.
• Advocate for clients and liaise with providers to secure appropriate care.
• Monitor progress and adjust care plans to improve outcomes.
• Collaborate with families, caregivers, and professionals to ensure holistic care.
