Job summary

Volunteers of America Los Angeles is seeking a Case Manager Specialist I to provide intensive case management to eligible clients, meeting participants weekly and coordinating services across the care continuum. This on-site role emphasizes advocacy, documentation, and strong collaboration with the service team.

The position requires completing comprehensive assessments, developing detailed case plans, and connecting clients with housing, health, and community resources while supporting

Qualifications

• Fingerprint clearance and background checks required.
• Two years total education/experience in social services or related field.
• VOALA Case Management training within 6 months of hire.

Responsibilities

• Conduct screenings and assessments to collect information for case plans.
• Provide service linkages and support systems to meet identified needs.
• Care coordination and case management of assigned caseload.
• Coordinate individualized planning with clients to meet short- and long-term goals.
• Advocate for clients before agencies and in the community.
• Engage clients in group activities and document sessions clearly.

Education
