Job summary

Volunteers of America Los Angeles seeks a Case Manager Generalist to provide on-site case management for low acuity clients, coordinating services and supporting access to resources. You will conduct screenings, develop plans, and advocate for clients within a collaborative team.

Requirements include at least 1 year in case management or a related field or a Bachelor's in Social Services. The position offers competitive benefits and requires on-site work in the VOALA program.

Qualifications

• Minimum 1 year of general experience in case management or similar social services role.
• Bachelor’s Degree in Social Services related field may substitute for experience.
• Ability to pass background checks and maintain confidentiality.

Responsibilities

• Conduct comprehensive screenings and assessments to gather functional, environmental, and health information.
• Develop individualized case plans and coordinate services to meet client needs.
• Advocate for clients before agencies and community resources.
• Coordinate care with team and monitor progress toward goals.
• Report suspected abuse as mandated.
