Qualifications

• Bachelor’s Degree in Behavioral Science or related field.

Responsibilities

• Responsible for the full client care cycle including assessing, planning, implementing, monitoring, and evaluating actions required to meet the clients’ health and human services needs.
• Work with offsite support to coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
• Maintain caseload and productivity by maintaining regular contact with clients to monitor and assess progress.
• Handle case assignments, draft service plans, review case progress and determine case closure with growing independence.
• Deliver ongoing services to clients including crisis intervention, advocacy, medication monitoring, transportation assistance, budgeting, housing assistance, and referral to community resources.
• Assist in helping clients achieve wellness and autonomy through supportive counseling and facilitating care coordination and information sharing.
• Develop and implement individualized client service plans and ensure compliance with standards and agency policies.
• Document all clinical activities to meet program, funder, and credentialing criteria timely and accurately.
• Participate in treatment team and assist in recommendations for client status changes.
• May lead educational groups or facilitate group curriculum.
