Job summary

Talbert House is seeking a Case Manager in Mount Orab, OH to oversee full client care cycles. You will assess, plan, implement, monitor, and evaluate services to meet health and human services needs.

Under minimal supervision, you will coordinate care, maintain caseloads, develop service plans, and document activities in accordance with agency policies. A degree in a behavioral health field is required, with preferred Bachelor's and a valid driver's license.

Qualifications

• Associate's degree in Human Services or related field required.
• Bachelor's degree in Social Work or related field preferred.
• Valid driver's license and reliable transportation required.
• Ability to learn case management principles and healthcare reimbursement requirements.
• Excellent organizational and communication skills.

Responsibilities

• Coordinate and provide care that is safe, timely, and client-centered under supervision.
• Maintain caseload and monitor client progress through regular contact.
• Draft service plans and review progress for potential case closures.
• Provide ongoing services including crisis intervention, transportation, and housing assistance.
• Document all clinical activities meeting program and credentialing criteria.
• Participate in treatment teams and support client education.
