Responsibilities

• Coordinate and provide care across the full client care cycle including assessment, planning, implementation, monitoring, and evaluation.
• Maintain caseload and monitor progress with regular client contact.
• Draft service plans and determine case closure in consultation with supervisor.
• Deliver ongoing services including crisis intervention, advocacy, medication monitoring, and linkage to resources.
• Document clinical activities to meet program and credentialing criteria.
