Qualifications

• Associate's degree in a related behavioral health/human services field is required.
• Bachelor's degree in Social Work or related behavioral health field is preferred.
• Must have or be eligible for Ohio Medicaid credentials as required by the role.
• Valid driver's license and reliable transportation are required.

Responsibilities

• Coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered.
• Maintain caseload and contact clients to monitor progress.
• Draft service plans and review progress with supervisor.
• Deliver ongoing services including crisis intervention, advocacy, housing assistance, and resource referrals.
• Document clinical activities to meet program and funder criteria.
• Participate in treatment teams and support client status decisions.
• Provide client education and facilitate group curriculum as required.
