Job summary

Colonial Group seeks a bilingual Case Manager to provide coordinated stabilization and time-limited case management to English- and Spanish-speaking clients in housing after exit from unsheltered homelessness or transitional shelter sites.

You’ll work with a close-knit team including case managers, navigators, nurses, and housing staff to ensure rapid exits into housing and coordinated follow-up for the first year.

Qualifications

• 1–2 years of experience working with homeless populations.
• Bilingual in English and Spanish.
• Experience with substance use or mental health challenges.
• Knowledge of housing resources and benefits programs.

Responsibilities

• Provide case management and crisis intervention.
• Coordinate care with housing navigators and other providers.
• Develop crisis and safety plans tailored to client needs.
• Assist with benefits applications and referrals.
• Document services in the client’s chart (EHR).
