Job summary

Delta-T Group Massachusetts, Inc. is seeking a temporary Housing Case Manager to conduct interviews and assessments for shelter guests, coordinate referrals, and develop targeted case plans. You will maintain HMIS records and support guests in accessing housing resources.

The role requires a health/social services background, bilingual skills (Haitian/Spanish preferred), and ability to initiate crisis interventions. Office location is Boston, MA with timely documentation expectations.

Qualifications

• College degree or GED, depending on experience.
• 1–2 years of health and human services work with homeless population.
• Bilingual in Haitian Creole or Spanish preferred.
• Basic proficiency in word processing using Windows, Word, and Google Apps.

Responsibilities

• Interview and assess shelter guests after five days and begin case management.
• Complete intakes, assess needs, identify factors affecting housing access and resources.
• Develop individualized case plans with goals and monitor progress weekly.
• Maintain HMIS records and document timely case notes and services provided.
