Job summary

The People Concern in Los Angeles seeks a Bilingual Case Manager to support clients experiencing homelessness through the 1603-Housing Retention program. The role emphasizes housing stability, trauma-informed care, and inter-agency coordination in a 100% onsite setting.

You will manage a caseload, conduct assessments, develop individualized plans, document progress, and collaborate with DMH/DHS partners to facilitate housing placement and retention. Fluency in Spanish is required.

Qualifications

• Proficient Spanish language skills to engage with monolingual Spanish-speaking clients.
• High School Diploma or GED or equivalent.
• Able to work in a high tolerance Harm Reduction model with clients experiencing multiple barriers.
• Valid CA Driver’s license, reliable car, auto insurance, and an acceptable driving record.
• Willing and able to transport clients in your personal vehicle.

Responsibilities

• Maintain a caseload of approximately 20 high-acuity Clients for intensive case management services.
• Develop relationships with Clients with a focus on facilitating independence and maintenance of improved physical and mental health.
• Collaborate with the LA County DMH and DHS programs to enhance coordination within the Clients’ treatment team.
• Engage Clients and implement interventions with an approach emphasizing Client choice and trauma-informed care.
• Assist Clients in applying for and obtaining all benefits to which they are entitled.
• Coordinate appointments, transportation, and follow-up services to improve access to care and resources.
• Directly transport Clients in your personal vehicle to agencies and providers in the greater Los Angeles area.
