Tpocc in Fresno, CA is seeking a Mental Health Specialist/Case Manager IV to support clients in accessing medical, housing, and social services. You will provide case management and rehabilitation services, collaborate with community partners, and ensure timeliness of documentation and billing.

You will work in settings such as client residences, schools, tele-health, and clinics, with bilingual Spanish communication preferred.

Competenze

• Bachelor’s degree in social work or counseling.
• 2. One year of related experience preferred.
• 3. Proficient in written and spoken English.
• 4. Valid driver’s license and transportation.
• 5. Ability to pass background check and TB test.

Mansioni

• Provide mental health services to children, youth and care providers in homes, schools, and clinics.
• Perform case management and rehabilitation services, including group and individual counseling as outlined in the Plan of Care.
• Collaborate with community partners and maintain timely documentation and billing.
• Meet monthly performance metrics and complete required trainings.
