Job summary

Turning Point of Central California, Inc. seeks a Dual Diagnosis Mental Health Specialist/Case Manager V to join the ACT/FACT team in Fresno. You will work on treatment plans, coordinate services for clients with complex needs, provide direct counseling, and maintain documentation.

Bilingual Spanish communication is preferred. Position is full-time with a strong emphasis on outreach, crisis response, and collaboration with a multidisciplinary team to support clients toward sobriety and recovery

Qualifications

• Bachelor’s degree from an accredited college or university in social work, psychology, vocational rehabilitation, criminal justice, or a related field.
• At least one year of experience in a mental health or human services related field.
• Current certified training in emergency first aid procedures preferred.
• Prefer ADP-approved substance abuse counseling certification or enrollment within six months of hire.
• Knowledge of substance abuse, addictions, and related disorders; able to lead educational groups.

Responsibilities

• Assist in the case management plan by screening, evaluating, and implementing activities per the treatment plan.
• Plan, coordinate, and facilitate services for Severe Mental Illness (SMI) clients.
• Work on a multi-disciplinary team providing integrated services (medications, therapy, housing, employment, etc.).
• Provide frequent contact with clients via community/field-based services and some office-based work.
• Deliver direct services including case management and rehabilitation skills.
• Educate clients through recovery, wellness, and sobriety groups.
• Refer clients to detox, residential treatment, or sober living as needed.
• Maintain case records per Medi-Cal guidelines and contract requirements.
