Job summary

Responsibilities include direct services, service planning, record-keeping, and coordination of services for severely mentally ill clients, with emphasis on timely reporting and compliance. A bachelor’s degree and valid driver’s license are required.

Qualifications

• Bachelor's degree in social work, psychology, vocational rehabilitation, criminal justice, or related field.
• 1+ year of experience in rehabilitation case management for families with barriers to independent community life.
• Knowledge of treatment approaches for Severe Mental Illness (SMI) and Dual-Diagnosis individuals.

Responsibilities

• Assist in implementing the IS SP case management plan under supervision.
• Maintain case records per Rehab option Medi-Cal guidelines and contract requirements.
• Provide direct services including group/individual counseling as outlined in ISSP and Plan of Care.
