Job summary

Firelands Health in Sandusky, OH, is looking for a clinician to provide community-based services to youth and adults with mental health and substance use disorders. You will perform case management, crisis stabilization, and Day Treatment services, following departmental guidelines and federal/state standards.

You will collaborate with a multidisciplinary team, engage patients in recovery, and help coordinate with natural supports to promote sustained well-being.

Qualifications

• Bachelor degree in psychology, social work, or related field.
• Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.
• Ability to effectively understand and communicate in both written and oral form.
• Punctual attendance at assigned work location is required.
• Ability to be mobile and possess a valid State of Ohio motor vehicle operator’s license and be insurable under Firelands’ auto policy.

Responsibilities

• Provide community-based services to youth and adults with mental health and substance use disorders under supervision.
• Perform scope of mental health/substance use case management: assessment, link with supports, symptom monitoring, advocacy, and recovery activities.
• Provide crisis support and stabilization, de-escalation, and safety planning with treatment team coordination.
• Deliver Day Treatment services as assigned by Program Coordinator.
• Ensure services comply with Medicaid/Medicare, HFAP, OhioMHAS, and CARF guidelines.
• Engage patients in treatment planning and support long-term recovery with natural supports.
