Qualifications

• Bachelor degree in psychology, social work, or related field is required.
• Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.

Responsibilities

• Under supervision, provides community-based services to youth and adults with mental health and substance use disorders.
• Provides case management services including ongoing assessment of needs and linkage with supports.
• Coordinates the ITP including services identified within and connections with natural and formal supports.
• Provides crisis support and stabilization, including de-escalation and safety planning.
• Delivers Day Treatment services to adults as assigned.
• Ensures services comply with Medicaid/Medicare, HFAP, OhioMHAS, and CARF guidelines.
• Engages in professional growth and quality initiatives to improve patient engagement and outcomes.
• Maintains accurate clinical documentation and supports revenue-generating tasks.
• Utilizes EMR and video-conferencing tools, and adheres to HIPAA and 42 CFR regulations.
• Acts as a team player, supporting colleagues and contributing to positive morale.
