Job summary

Firelands Health in Sandusky is seeking a community-based mental health and substance use case manager to support youth and adults under supervision. You will assess needs, coordinate services, and promote recovery through goal-oriented treatment planning.

Responsibilities include crisis support, discharge planning, Day Treatment, and outreach, ensuring care aligns with Medicaid/Medicare, HFAP, OhioMHAS, and CARF standards. A Bachelor’s degree and relevant certifications are required.

Qualifications

• Bachelor’s degree in psychology, social work, or related field.
• Heart Saver, Aegis Crisis Prevention, and First Aid certification within 6 months of hire.
• Ability to effectively understand and communicate in both written and oral form.
• Punctual attendance at assigned work location is required.
• Valid State of Ohio driver’s license and insurable under Firelands’ auto policy.
• Ability to work safely and follow OSHA, CDC guidelines and policies.

Responsibilities

• Provides community-based services to youth and adults with mental health and substance use disorders.
• Performs ongoing assessment of needs and coordinates services identified within the treatment plan.
• Provides crisis support and stabilization, including safety planning and coordination with the treatment team.
• Delivers Day Treatment services as assigned by the Program Coordinator.
• Ensures services comply with Medicaid/Medicare, HFAP, OhioMHAS, and CARF standards.
• Promotes recovery through collaborative treatment planning and engagement with natural supports.
