North Central Mental Health Services, Inc. is seeking a Behavioral Health Case Manager in Franklin County to provide compassionate, community-based care. You will develop care plans with clients, coordinate services, advocate across systems, monitor progress, and intervene in crises as needed.

The role emphasizes education, collaboration, documentation, and culturally sensitive support. The position offers a full-time schedule, competitive pay at $25.00/hour, and a comprehensive benefits package

Qualifications

• Bachelor’s degree required in social work, psychology, behavioral health or related field.
• Demonstrated compassion and dedication to helping people.
• Organized and detail-oriented with strong resource management.
• Familiarity with community resources (preferred).
• Experience in mental health or case management (preferred).
• Ability to provide 1,248 patient service hours annually.
• Valid Ohio driver's license with reliable transportation.

Responsibilities

• Care Plan Development: develop personalized treatment plans with clients, families, and healthcare professionals.
• Service Coordination: connect clients to therapy, housing, employment support, and recovery programs.
• Advocacy: advocate for clients within healthcare, legal, and social service systems.
• Ongoing Monitoring: track progress, ensure treatment adherence, adjust plans as needed.
• Crisis Intervention: provide immediate support during emergencies.
• Education & Support: empower clients with mental health information and resources.
• Collaboration: liaise between healthcare professionals, social workers, and benefit providers.
• Documentation & Compliance: maintain EMR records meeting standards.
• Empowerment & Cultural Sensitivity: deliver respectful, inclusive services.
