Job summary

Anew Behavioral Health in Ohio is seeking a Behavioral Health Case Manager to coordinate care for clients with mental health and substance use challenges. You will assess needs, create service plans, and work with multidisciplinary teams to support recovery and independence.

Qualifications include a high school diploma (Human Services degree preferred), CPR certification (preferred), and experience in behavioral health. Flexible scheduling and community involvement are encouraged.

Responsibilities

• Coordinate and provide client-centered care that is safe, timely, effective, and efficient.
• Manage case assignments, draft service plans, and review progress.
• Advocate for clients in decisions about their treatment options.
• Collaborate with therapists, clients, and families to meet behavioral health needs.
• Support clients in achieving independence and daily living skills.
• Perform other duties as assigned and maintain documentation per CARF, CMS.
