Job summary

Community Health Alliance is seeking a compassionate Case Manager to provide direct support to individuals with mental health conditions, coordinating care and teaching daily living skills. The role is community-based with home visits and collaboration with providers, families, and supports.

Qualifications

High School Diploma required; preferably 3+ years in behavioral health or a related field.
Bachelor’s degree in a social sciences field or related area considered an asset.

Responsibilities

Coordinates assessments, treatment planning or crisis intervention services.
Facilitates healthcare, behavioral health and other services as part of care coordination.
Provides training and linkage to community resources.
