Job summary

Horizon Health and Wellness, Inc. is seeking a Care Manager to ensure coordinated delivery of services for clients in our behavioral health programs in Arizona.

You will advocate for clients, manage caseloads, document plans, and collaborate with clinical teams while maintaining timely communication and compassionate patient care in a fast-paced setting. This full-time role requires 21+ years old, a valid driver's license, and flexibility to work evenings as needed.

Qualifications

• HS diploma or GED plus 4 years in Behavioral Health or combined Behavioral Health education and experience to meet Behavioral Health Technician qualifications.
• One year case management experience strongly preferred.
• Certification in first aid, CPR and Crisis De-escalation (training will be offered if do not possess upon hire).
• Must be at least 21 years of age or older.
• Possess valid drivers license with acceptable driving record and reliable transportation.
• Prior experience in the behavioral health setting is strongly preferred.
• Demonstrated organizational abilities to maintain communication, coordination, and connections with clinical team participants.
• Willingness to work evenings as needed.

Responsibilities

• Care Managers main purpose is to ensure their client caseload is provided with the necessary services they need, in a coordinated and effective manner.
• Responsible for the caring and compassionate delivery of case management, advocacy and other supportive services to ensure appropriate coordination of services for clients.
