Job summary

Horizon Health and Wellness, Inc. is seeking a Care Manager to coordinate and deliver compassionate case management and advocacy for clients in central and southern Arizona.

You will ensure services are coordinated and tailored to client needs, while maintaining strong collaboration with the clinical team. The role requires a HS diploma or GED with behavioral health experience, case management background, and valid driver’s license.

Qualifications

• HS diploma or GED plus 4 years in Behavioral Health to meet Behavioral Health Technician qualifications.
• One year case management experience preferred.
• Certification in first aid, CPR and Crisis De-escalation.
• Must be at least 21 years of age and have a valid driver's license.
• Ability to create plans and summaries based on client needs.
• Willingness to work evenings; strong time-management and organization while in a fast-paced environment.
• Fingerprint clearance card required; pre-employment drug and background check.
• TB screening and annual updates; CPR/First Aid certification to be provided if not possessed.
• Physical, mental and cognitive ability to perform tasks without compromising safety.
• Bilingual English/Spanish is preferred.
