Job summary

Horizon Health and Wellness, Inc. in Yuma, AZ is seeking a Full-Time Care Manager to coordinate client services and deliver compassionate case management within a behavioral health setting.

The role requires a HS diploma or GED with behavioral health experience, plus strong communication, organization and advocacy skills. A valid driver’s license and background check are required; bilingual English/Spanish is a plus.

Qualifications

• HS diploma or GED plus 4 years in Behavioral Health or combined Behavioral Health education and experience to meet Behavioral Health Technician qualifications.
• One year case management experience strongly preferred.
• Ability to create plans and summaries based on client needs.
• Certification in first aid, CPR and Crisis De-escalation (training will be offered if do not possess upon hire).
• Must obtain and maintain a fingerprint clearance card and pass a pre-employment drug and background check.

Responsibilities

• Ensure client caseloads are provided with necessary services in a coordinated and effective manner.
• Deliver compassionate case management, advocacy and supportive services.
• Coordinate and communicate with the clinical team to maintain service continuity.
