Qualifications

• HS diploma or GED plus 4 years in Behavioral Health or combined Behavioral Health education and experience to meet Behavioral Health Technician qualifications.
• One year case management experience strongly preferred.
• Must obtain and maintain a fingerprint clearance card and pass a pre-employment drug and background check.
• Must be at least 21 years of age or older.
• Possess valid driver's license with acceptable driving record and reliable transportation.

Responsibilities

• Care Managers ensure client caseload is provided with necessary services in a coordinated and effective manner.
• Deliver caring and compassionate case management, advocacy, and other supportive services.
• Coordinate services for clients and maintain communication with clinical team participants.
