Job summary

Solutions for Care is seeking an Adult Protective Services Case Worker in North Riverside, IL. The role encompasses investigation of abuse, neglect and exploitation among seniors and disabled adults, ensuring safety and care planning in the least restrictive setting.

Requirements include a BA/BS in a related field, bilingual Spanish/English preferred, and a valid driver's license with a vehicle for local travel.

Qualifications

BA/BS degree in Social Science, Social Work, Gerontology, or related field.
Bi-lingual Spanish/English preferred.
Valid driver’s license and a vehicle for local travel.
Strong organizational and time management skills.
Criminal background check required.

Responsibilities

Investigate reports of Abuse, Neglect, Financial Exploitation, or Self-Neglect of eligible adults.
Ensure safety and basic care needs are met in the least restrictive environment.
Attend required APS training (Phase I/II and simulations).
Maintain accurate case notes, reports, and documentation with time management.
Communicate professionally with clients, families, and partner agencies.
Respect client self-determination and maintain confidentiality.
Complete other assignments as directed by the APS supervisor.
Travel to home visits, hospitals, trainings, and meetings as needed.
